By completing this form, I give consent for my child to participate in all activities included with GSC Kids Camp 2023 including travel to The Cove Waterpark and Camp Copass in camp staff or Glory of Zion vehicles.
I understand that if an emergency arises, efforts will be made to contact me. In case of emergency, I hereby give permission the the physician selected by GSC Camp Staff to hospitalize, secure proper treatment for, and order any medical care necessary for my child while attending GSC Kids Camp 2023.
I waive all claims and rights to prosecution against Glory of Zion International Ministries, Inc. and Global Spheres Center and their representatives during GSC Kids Camp 2023 for any injuries that may be sustained by my student and agree to indemnify and hold said person(s) free and blameless from liability therefore.
I understand that a $25 non-refundable deposit is due to secure my child's registration.
I understand that remaining payment of $75 is due by June 1, 2023.
